MTPL insurance – party at fault

1. First of all, report the incident on the basis of an accident record (signed by both parties) or confirmation by telephone from the police, filling in an online form or a written form (send it to email: registrace@slavia-pojistovna.cz, or by post)
2. The policyholder will receive claim number within 24 hours via a text message (we will also communicate it on the information line)
3. Subsequently, it’s necessary to submit documents: a copy of the vehicle registration, a copy of your driving licence, a copy of the accident record (by post, through a branch or by email: lpu@slavia-pojistovna.cz; include claim number in the email subject)

MTPL insurance – injured party

1. First of all, report the incident on the basis of an accident record (signed by both parties) or confirmation by telephone from the police, filling in an online form or a written form (send it to our email: registrace@slavia-pojistovna.cz, or by post)
2. The injured party will receive claim number within 24 hours via a text message (we will also communicate it on the information line)
3. After assigning the claim, the employee will contact the injured party regarding photos being taken by an officer; to speed-up the process, call the information line and make an appointment by telephone
4. Subsequently, it’s necessary to submit documents: a copy of the vehicle registration, a copy your driving licence, a copy of the accident record, budget application with account number OR invoice with the power of attorney (by post, through a branch or by email: lpu@slavia-pojistovna.cz; include claim number in the email subject)

Motor accident insurance / windscreen

1. First of all, report the incident by telephone, filling in an online form or a written form (send it to email: registrace@slavia-pojistovna.cz, or by post)
2. The injured party will receive claim number within 24 hours via a text message (we can also communicate it on the information line)
3. Subsequently, it’s necessary to submit documents: a copy of the vehicle registration, a copy of your driving licence, budget application with account number OR invoice with the power of attorney, and photo documentation in case of a windscreen (by post, through a branch or by email: lpu@slavia-pojistovna.cz; include claim number in the email subject)

Other type of insurance

1. First of all,  report the incident by filling in the appropriate written form according to the type of insurance (send it to our email: registrace@slavia-pojistovna.cz, or by post)
2. The injured party will receive claim number within 24 hours via a text message (we will also communicate it on the information line)
3. Subsequently, it’s necessary to submit documents on the basis of an invitation sent by the claims adjuster by post: by post, through a branch or by email: lpu@slavia-pojistovna.cz; include the claim number in the subject of email
4. If it’s necessary to have an officer take photos, the injured party will be contacted by the insurance company employee
